Friendly, quick and professional. You don't pay for your form. You do have to go to second office to notarize forms. Both offices are same. Main office behind Quality Inn.

Drove 4 hours .time on door says 9am to 3pm got there at 9 still closed they open @10am. If your there to renew your CIB ID. Please make sure you get a Money order for $17.00 before you head there.and if your wanting to get a CIB ID they are currently not giving them out untill further notice .overall experience was ok except for the hours .

Stay far away from this place! I too have been dealing with these people for a year and 3 months now. My case worker is Alicia and she should be fired! She is unprofessional and very rude. I've spoken to the manager Ronald several times and still nothing has been done. Someone higher up needs to do something. I've pleaded with them to send me my child's birth certificate back. They say they will call. They say it will be sent out but I've received nothing! I too will be taking this further up. This is not ok! This office needs to be looked into.

I would give this not even a star rating. It’s been over a year since I have been trying to get my CIB. Luckily they did send me back my birth certificate with an official application. I’ve been trying to get in touch with someone regarding but I get the run around. Sent multiple emails and have tried calling numerous times the phone won’t even go all the way through so I am unable to talk to someone regarding situation. If you plan on enrolling for your CIB start sooner than later as it may take years!!! Super frustrating I’m not sure who’s in charge but something needs to change it should not be this much of a hassle to enroll for our CIB.

Location has changed since I've been to N.N. vital records in Windowrock AZ. They are now located behind Quality inn in amber color building. Hours are 9:00am-2:30pm (New enrollment 9:00am-1:00pm) fyi I went in today to enroll my newborn son. I was greeted with friendly staff who help me with my process. I had all my documents in hand so it only took an hour to walk out of there with CIB in hand. I was nervous with all the one star reviews but gladly that wasn't the case for me.

I wouldn't even give them a slap on the back for a good job, seems like you have too give a star. Charmaine was very rude over the phone. Ask to speak with the supervisor and was told he's not in. No help at all. Very unprofessional over the phone. They need customer service training.

The staff was so friendly and helpful, and quick and efficient. The only reason I didn't give 5 stars, the system of signing in was pointless. They don't look at the sign-in sheet and call the next person, they just asked who is next, and people kept going before me even though I had signed in before them. It wasn't a big deal, since they moved through everyone rather fast, it was just a bit disorganized and chaotic.

The “assistant manager” Alicia is completely unprofessional and rude. I’ve called in on a few different occasions with simple inquiries. Alicia has laughed at my face, laughed at me, to co workers, hung up in my face, and more recently today she stated “you relinquished your rights with Navajo Nation and are no longer a member of this tribe, good bye” and disconnected the call when I asked her a question about a letter I received in the mail. This girl has serious internal/ mental issues to act out in such an unprofessional manner to people who come to her regarding simple matters. She claims to be an “assistant manager” and I am not the first person to complain about her. I have already complained to the manager Ronald about this and obviously nothing was ever done about it because she continues to act like this. I will be escalating this matter to the presidents office! I feel bad for the people that have to work with her. People like that make the entire tribe look bad especially with the position that she holds. Like have some decency to yourself and others! How sad that some people’s self image and self respect is so poor..
